To Register for the UPYSST:
- Download the Registration Forms, Complete them, then them into the Upper Perkiomen Valley YMCA Annex
- Go to the Upper Perkiomen Valley YMCA Annex and complete the forms
ALL PAYMENTS ARE TO BE PAID AT THE UPV YMCA ANNEX and at the 2011 Kick-off Event. Checks should be made payable to: "Freedom Valley YMCA".

The two above forms are required by the UPV YMCA and get turned into the UPV YMCA Annex. THEY ARE MANDATORY FOR THE ATHLETE TO BE ELIGIBLE TO PARTICIPATE IN THE 2010 SUMMER SEASON!
Photo Release Form ALL PARTICIPANTS, MUST ALSO HAVE A PHOTO RELEASE FORM COMPLETED. THE FIRST FORM BELOW IS MANDATORY FOR ALL SWIMMERS, IF "deny" IS CHECKED, THE SECOND DOES NOT HAVE TO BE COMPLETED. IF THE FIRST IS CHECKED AS "grant" THEN THE SECOND FORM MUST BE FILLED OUT. ANY QUESTIONS CONTACT ZAK KIBLER (zkibler@upysstotters.org). Any Swimmer that is currently 18 years old, or that will turn 18 anytime during the season must also fill out another release form.
